Kindred to close Greenfield nursing home

Kindred Transitional Care and Rehabilitation-Milwaukee in Greenfield, formerly known as Mount Carmel, announced today it is seeking permission from the Wisconsin Department of Health Services to close, according to a letter sent to residents and family members.

 
The nursing home at 5700 W. Layton Ave. is leased by Kindred from Ventas.

“The decision was based on several factors, including the fact that Kindred leases this facility from Ventas and a joint decision was made between the landlord and Kindred to close the facility. In addition, Kindred Transitional Care and Rehabilitation-Milwaukee’s size, age and design is no longer consistent with the current consumer preferences and needs for today’s health care delivery system,” the letter stated.

Per requirements, the closure cannot occur until after the completion of the approved resident relocation plan which must be at least 120 days from the approval of the plan.

The facility has 473 licensed beds and an average of about 300 residents.
