Hospice agency collecting nominations for ‘Touching Lives’ Award

Horizon Home Care and Hospice, a Brown Deer-based nonprofit organization that functions as a full-service home care agency and hospice provider, is making a call for nominations for its annual “Touching Lives” Award.

 

The award, presented by the nonprofit, recognizes individuals who have made a significant difference in the lives of others.

Individuals eligible for recognition will have demonstrated:

*At least 20 years of community service
*A deep commitment to serving others that extends beyond expectations
*Innovative initiatives to develop an organization or project or strengthen an existing one
*Leadership skills in compelling others to collaborate for a common cause
*Proven results in their work, whether in health care, human services, or community projects
*Lasting change that benefits others through their efforts

Award winners will be honored in mid-February at the organization’s 2014 Matters of the Heart Gala, which benefits the Horizon’s Grief Resource Center. Award recipients are given hand-blown glass works of art designed and created by artist Daniel Ostrov.

Past “Touching Lives” honorees include former Wisconsin First Lady Sue Ann Thompson and executives Leo Brideau, Bill Petasnick, Paul Westrick and Pamela Parker.
