Features: 13 dental suites; 24 primary care rooms; three specialty care exam rooms; six OB/GYN exam rooms; administrative office space; phlebotomy laboratory; health education center; and a partially finished basement that can be built out for future expansion needs, such as radiology/mammography and integrated behavioral health.

Services provided: Primary health care; family medicine; internal medicine; pediatrics; financial counseling; onsite laboratory services; women’s health services; education programs; dental services; translation and interpretation; transportation assistance; social work; and emergency care coordination.
