Fatal industrial accident at ATI Ladish

Company has had 13 safety violations since 2007

A 49-year-old Oak Creek man was killed in an industrial accident Thursday at ATI Ladish Forging in Cudahy.

A spokesperson for Ladish confirmed the death to BizTimes media partner WISN 12, but declined to offer specifics. The Milwaukee County medical examiner responded to the accident around 2:30 p.m. A 48-year-old Cudahy man has been hospitalized, according to WISN 12.

Ladish, on the 5400 block of South Packard Avenue, makes engineered metal components for jet engines, aerospace and industrial markets.

Christine Zortman, area director of the Occupational Safety and Health and Administration’s Milwaukee office, said an OSHA investigator was at the site.

OSHA has cited Ladish for 13 safety violations since 2007. The last violation was in 2014, involving a forging hammer.

Zortman said she is unsure if this is the same piece of equipment was involved in today’s accident.

“We will be opening up an investigation and inspection to determine if any violations of the Occupational Safety Act was violated,” Zortman said.
