The Wisconsin Department of Health Services has cited an Oshkosh mental health facility in connection to the June 5 death of a patient, a department spokeswoman confirmed.
Erik Haak died June 5 after an incident at the Winnebago Mental Health Institute, where he had been a patient, Winnebago County Coroner Barry Busby said Thursday, Aug. 13.
