The 23 recognized physicians are internal medicine or family practice physicians. They see patients at Medical Associates Health Centers in Menomonee Falls, Waukesha, Germantown, Sussex and Hartford. Recognition status is based on data which demonstrates that each physician’s performance at Medical Associates meets or exceed the program’s key diabetes care measures, including eye exams, blood pressure tests, nutrition therapy and patient satisfaction, among others.
